assessment, we will ask you what assistance you would like. We will
then discuss your options and help you devise a Plan of Care.
This will determine who will assist you in your home, what duties they
will perform, and how often. The following may help you understand how we
classify our caregivers. We are committed to partnering with your
physician and other care providers to enhance the quality of your life.
Home Health Care Services include...
Check-In Service
This is an new and exclusive, low cost
alternative to traditional home care!
Check-In Service is customized to fit your specific needs. If
you do not require assistance with the activities of daily living, yet
your family and friends are concerned about you being on your own, this is
the service for you. We arrange to phone you 2 times a week (or more) and
make one personal visit each week, just to make sure you are doing well.
It brings great comfort to know someone nearby cares and will check in on
a regular basis. You will come to enjoy our visits and look forward to our
brief calls. And, if you ever require additional assistance, you will
already be familiar with the Aker Kasten Family which stands ready with
additional resources. This is a highly individualized service, so call for
details.
Homemaker/Companion
Services
These caregivers provide assistance with domestic duties and household
maintenance such as light housekeeping, grocery shopping, meal
preparation, laundry & linen changes, and escorting to appointments.
Home Health Aide
These caregivers are able to provide more personal care. They often
provide assistance with activities of daily living including personal
hygiene, bathing & dressing, bathroom assistance & incontinent care plus
transfers from bed to chair, prescribed exercises, and supervision of self
administered medication.
Private Duty Nursing
These professionals provide individualized service at home or hospital,
including complete patient assessment, home safety assessments, nutrition
planning assistance, geriatric depression screening, medication compliance
monitoring, and patient & family training.

Case Management
Our Senior Advocate, Heidi, Smith, can assist with evaluation,
planning of, and arrangement for, appropriate services. This provides a
very comprehensive approach to patient care. With extensive training in
geriatric issues, Heidi educates patients and their families on care
options to optimize patient outcomes. Heidi has a sincere sensitivity in
affirming dignity and enhancing quality of life for all.
Insurance Provider Assistance
To alleviate the burden of excessive paperwork and stress, we will
assist you with long term care insurance inquiries. While we are a private
duty agency, we will assist you in filing claims with your long-term care
insurer. Typically, patients pay our invoices while we assist them in
obtaining reimbursement from their insurance company. Some individuals
prefer to file an Assignment of Benefits form authorizing their insurer to
pay Aker Kasten Home Health directly. We are ready to help you access, in
a trouble-free way, the benefits you have paid for!
